Evaluation of operative complications in obese patients who underwent laparoscopic sleeve gastrectomy: Single-center experience

INTRODUCTION: The complications experienced during and after surgery in patients who underwent laparoscopic sleeve gastrectomy for obesity treatment in 1 clinic were analyzed.
METHODS: The demographic and clinical data of all patients who underwent bariatric surgery with the diagnosis of obesity in the Kahramanmaras Sütçü İmam University Faculty of Medicine General Surgery Department between April 2008 and June 2016 were examined retrospectively by reviewing the hospital information system and patient files. Complications experienced during and after the operation were examined in 431 patients who underwent laparoscopic sleeve gastrectomy. The complications were divided into peroperative and postoperative types. Deaths within the first 30 days were considered operational mortality.
RESULTS: Hemorrhage was detected in 13 patients and anastomosis leakage in 2 patients as a perioperative complication. In 51 patients, 1 or more complications developed postoperatively. In all, 20 patients experienced hemorrhage, 12 had anastomotic leakage, 12 had pneumonia, 10 had intra-abdominal abscess, 5 had a wound infection, 2 had a pulmonary embolism, and small bowel necrosis, portal vein thrombosis, myocardial infarction, and small bowel perforation was each seen in 1 patient. Postoperative mortality was observed in 6 patients (1.39%).
DISCUSSION AND CONCLUSION: Laparoscopic sleeve gastrectomy is currently the most common, popular surgical procedure in the appropriate obese patient group in terms of results. Like any surgical procedure, however, it cannot be said that laparoscopic sleeve gastrectomy is completely free of the risk of complications.

Obez Hastalara Uygulanan Laparoskopik Sleeve Gastrektominin Operatif Komplikasyonlarının Değerlendirilmesi: Tek Merkez Deneyimi

GİRİŞ ve AMAÇ: Kliniğimizde obezite tedavisinde laparoskopik sleeve gastrektomi uygulanan hastalarda cerrahi sırasındaki ve sonrasındaki komplikasyonları araştırmak
YÖNTEM ve GEREÇLER: Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda Nisan 2008 - Haziran 2016 tarihleri arasında obezite tanısı ile bariyatrik cerrahi uygulanan tüm hastaların demografik ve klinik verileri hastane bilgi sistemi ve hasta dosyaları gözden geçirilerek retrospektif olarak incelendi. Laparoskopik sleeve gastrektomi uygulanan 431 hastada operasyon sırasındaki ve sonrasındaki komplikasyonlar incelendi. Komplikasyonlar peroperatif ve postoperatif olarak ayrıldı. İlk 30 gün içindeki ölümler operasyonel mortalite olarak kabul edildi

BULGULAR: 13 hastada hemoraji ve 2 hastada anastomoz kaçağı perioperatif komplikasyon olarak saptandı. 51 hastada bir ya da daha fazla komplikasyon postoperatif olarak gelişti. Hastaların 20'sinde hemoraji, 12'sinde anastomoz kaçağı, 12'sinde pnömoni, 10'unda intraabdominal abse, 5'inde yara yeri enfeksiyonu, 2'sinde pulmoner emboli, ince barsak nekrozu, portal ven trombozu, miyokard infarktüsü, bir hastada ince barsak perforasyonu vardı. Postoperatif mortalite 6 hastada (% 1,39) görüldü.
TARTIŞMA ve SONUÇ: Laparoskopik sleeve gastrektominin, günümüz koşullarında elde edilen sonuçlar açısından uygun obez hasta grubunda en sık uygulanan ve popüler cerrahi prosedür olduğunu söyleyebiliriz. Fakat herhangi bir cerrahi prosedür gibi, laparoskopik sleeve gastrektominin tamamen masum bir prosedür olduğunu söyleyemeyiz.
