Our Laparoscopic Repair Experience In Ventral Hernias

INTRODUCTION: Evaluation Of techniqual properties and outcomes of laparoscopic ventral hernia repair.
METHODS: Laparoscopic repair Was applied to 22 ventral hernia cases in Istanbul University, Cerrahpa¥a Medie-al Faculty, Department Of General Surgery, section of Gürkan C between January 1998-January 2000.
RESULTS: : Total number of cases Were 22. There were I I umblical hernia cases with three recurrences, 10 incisiOnal hernias and one epigastric hernia. Six of the incisional hernia cases were originated from Phannenstiel incision, one from distal part of the upper midline incision, one from proximal part of lower midline incision and two of the cases originated from right transrectal incisions. Nineteen Of the cases were female and three Of them were male. The youngest case Was 37 years Old and the oldest was 70 years old with the mean age of the series as 53.95±2.03 years. In two of the cases polytetrafloroetylene mesh of the appropriate size was used for repair of the hernia and polipropilene mesh was used in the remaining cases. Six of the cases were discharged from the hospital in the first postoperative day and the others were discharged in the second postoperative day. In a case of recurrent umblical hernia, seroma around the umblicus was detected in the early postoperative period. Cases were followed up between 6-12 month with a mean of 9.14 ± 0.54 months by regular follow-ups at 1st, 3rd, 6th and 12th months. No recurrence was observed during the follow up period.
DISCUSSION AND CONCLUSION: Laparoscopic ventral hernia repair with mesh application is a preferable and safe tecnique in selected cases with advantages of 0% mortality rate, 4.54% morbidity rate (1 / 22), shorter hospital stay, satisfactory cosmetic results and rapid return to daily activities.

Ventral fıtıklarda laparoskopik onarım deneyimimiz

GİRİŞ ve AMAÇ: Laparoskopik ventral fıtık onarımının teknik özelliklerini ve sonuçlarını incelemek.
YÖNTEM ve GEREÇLER: Ocak 1998 ve Ocak 2000 yılları arasında İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Genel Cerrahi Anabilim Dalı Gürkan C servisinde toplam 22 ventral fıtık olgusuna laparoskopik onarım uygulandı.
BULGULAR: Olguların 3'ü nüks olmak üzere 11'i umblikal, 10'u insizyonel, 1'i epigastrik fıtık idi. İnsizyonel fıtık olgularının 6'sı Phannenstiel, 1'i göbek üstü median insizyon distalinden, 1'i göbek altı median insizyonun proksimalinden, 2'si sağ transrektal insizyon yerlerinden gelişmişti. Olguların 19'u kadın, 3'ü erkekti. Serinin en genci 37, en yaşlısı 70, ortalama yaş 53.95 ± 2.03 idi. Olguların 2'sine uygun boyutlarda politetrafloroetilen mesh, diğerlerine polipropilen mesh ile fıtık onarımı yapıldı. Olgulann 6'sı ameliyat sonrası birinci gün, diğerleri ameliyat sonrası 2. gün taburcu edildiler. Nüks umblikal fıtığı olan bir olguda ameliyat sonrası erken dönemde göbek bölgesinde seroma tesbit edildi. Olgular en az 6 en £0k 12 ay, ortalama 9.14 ± 0.54 ay olmak üzere ameliyat sonrası l, 3, 6 ve 12. aylarda rutin kontrollere çagrıldılar. Takip süresince nüks tesbit edilmedi.
TARTIŞMA ve SONUÇ: Laparoskopik teknikle ventral fıtıklarda mesh uygulaması mortalite, (1/22) %4.54 morbidite oranları, kısalmış hastanede kalış süresi, kozmetik olarak daha tatminkar sonuçlar vermesi ve hastanın günlük aktiviteye dönüş zamanının daha kısa sürede gerçekleşmesi nedeniyle seçilmiş vakalarda tercih edilebilecek güvenilir bir yöntemdir.
