Swyer syndrome or pure gonadal dysgenesis is a state in which the karyotype is 46, XY and the phenotype is female. The main complaint of the patients is usually primary amenorrhea. Secondary sex characteristics are usually absent due to lack of estrogen. The risk of malignant transformation from the dysgenetic gonads is increasing with age and the gonadectomy should be performed promptly after the early diagnosis. In the absence of disseminated disease or huge tumor the preferred surgical technique is the laparoscopic removal. A laparoscopic gonadectomy was performed to a 16 years old patient with Swyer syndrome. On pathological examination it was found gonadoblastom in one gonad and gonadoblastom with dysgerminoma in the other. As the disease is restricted in the gonad it was decided to follow her without any adjuvant therapy and she is disease free since nine months.

Disgenetik gonadlardan germ hücreli tümör gelişme riski yaşla birlikte arttığından, bu hastalarda erken tanıyı takiben zaman yitirmeden gonadektomi yapılmalıdır. İleri evre hastalık ya da iri bir tümör gibi nedenlerin dışında tercih edilecek cerrahi yöntem laparoskopidir. Swyer sendromu olan, 16 yaşındaki bir hastaya laparoskopik gonadektomi yapılmıştır. Patolojik incelemede bir gonadda sadece gonadoblastom, diğer gonaddaysa gonadoblastomla birlikte disgerminom bulunmuştur. Gonada sınırlı hastalığı olduğundan
takip kararı verilen hastanın dokuz aylık takibi sorunsuz geçmiştir.
