Appendectomy with a single incision laparoscopic surgery port made from a plastic bottle

INTRODUCTION: Although minimally invasive surgery offers great advantages to patients, it has certain disadvantages, such as dependence on technology, it is not always easily accessible, and greater expense. The aim of this study was to present a simple and inexpensive new technique for performing the minimally invasive surgery of a single-port laparoscopic appendectomy.
METHODS: The data of 6 patients who underwent single-port laparoscopic appendectomy using the mouth of a plastic bottle and a glove between January and November 2017 were evaluated retrospectively.
RESULTS: The mean age of the patients was 22.5±2.73 years (range: 19–27 years). Three patients were male (50%) and 3 patients were female (50%). The mean duration of the operation was 61.8±5.52 minutes (range: 53–68 minutes). Two patients (33.3%) needed an additional port because manipulation could not be achieved and a laparotomy was performed on 1 (16.6%) of these patients. There were no complications except for a wound site infection in 1 patient.
DISCUSSION AND CONCLUSION: While minimally invasive single-port laparoscopic surgery is successfully performed, it is a method that has not yet become the gold standard, has a fairly long learning curve, and is difficult to access due to financial and technical reasons. Simpler and cheaper methods using the same principle can be applied successfully, provided that they are performed by experienced hands.

Plastik Şişeden 'Single İncision Laparoscopic Surgery' (Sils) Portu İle Apendektomi

GİRİŞ ve AMAÇ: Minimal invaziv cerrahi her ne kadar hastalara çok büyük avantajlar sunsa da, teknolojiye bağımlı olması, kolay ulaşılabilir olmaması ve daha pahalı olması gibi dezavantajlara sahiptir. Minimal invaziv cerrahi uygulanmak üzere tek porttan Laparoskopik apendektomi gerçekleştirmek üzere basit ve ucuz olan yeni tekniğimizi sunmayı amaçladık.
YÖNTEM ve GEREÇLER: Ocak - Kasım 2017 tarihleri arasında plastik şişe ağzı ve eldiven kullanılarak tek port laparoskopik apendektomi uygulanan 6 hastanın verileri geriye dönük olarak değerlendirildi.
BULGULAR: Hastaların ortalama yaşı 22.5±2.73 yıl (19-27) olup üç hasta erkek (%50) ve üç hasta kadın (%50) idi. Ortalama ameliyat süresi 61.8 ± 5.52 dakika (53-68) idi. İki hastada (%33.3) yeterli manüpilasyon sağlanamadığı için ek port gereksinimi oldu ve bu hastaların bir tanesine laparatomi (%16.6) yapıldı. Bir hastada görülen yara yeri enfeksiyonu dışında bir komplikasyon izlenmedi.
TARTIŞMA ve SONUÇ: Tek porttan minimal invaziv Laparoskopik cerrahi başarı ile uygulanmakta olan ancak henüz altın standart haline gelmemiş,uzun sayılabilecek öğrenme eğrisine sahip, mali açıdan ve teknik açıdan ulaşması zor yöntemlerdir. Aynı prensibin kullanıldığı daha basit ve ucuz yöntemler, tecrübeli ellerde olmak şartıyla başarı ile uygulanabilir.
