Staple line bleeding control with monopolar cautery in laparoscopic sleeve gastrectomy

INTRODUCTION: During a sleeve gastrectomy, cauterization of the staple line for bleeding control is not recommended due to the worry of weakening the staple line. This study is an evaluation of sleeve gastrectomy outcomes in a cohort where all staple line bleeding control was achieved using monopolar cautery.
METHODS: Between July 2014 and February 2019, monopolar cautery was used as the only method of staple line bleeding control following 187 sleeve gastrectomies. The rate of staple line leaks as well as intra-abdominal and intraluminal (gastrointestinal) bleeding was examined.
RESULTS: A total of 144 women and 43 men were enrolled in this study (mean age: 34.1 years; mean body mass index: 43.6 kg/m2). There were 5 leaks from the staple line (2.6%) and 7 cases of gastrointestinal bleeding (3.7%). There was no instance of mortality.
DISCUSSION AND CONCLUSION: Staple line bleeding control with monopolar cautery during sleeve gastrectomy is a feasible, simple, inexpensive, and relatively safe method. Randomized trials are needed to compare monopolar cautery with other staple line bleeding control methods.

Laparoskopik sleeve gastrektomide monopolar koter ile stapler hattı kanama kontrolü

GİRİŞ ve AMAÇ: Sleeve gastrektomi sırasında, stepler hattını zayıflatmasından endişelenildiği için kanama kontrolünde stapler hattının koterizasyonu önerilmez. Bu retrospektif çalışmada, tüm stapler hattı kanama kontrollerinin monopolar koter yapıldığı laparoskopik sleeve gastrektomi sonuçlarımızı değerlendirdik.
YÖNTEM ve GEREÇLER: Temmuz 2014 ile Şubat 2019 arasında, laparoskopik sleeve gastrektomi yapılan 187 hastada stepler hattı kanama kontrolünde tek yöntem olarak monopolar koter kullanıldı. Stapler hattı kaçaklarını ve intraabdominal ve intralüminal (gastrointestinal) kanama oranlarını inceledik.
BULGULAR: Çalışmaya toplam 144 kadın ve 43 erkek dahil edildi (ort. Yaş 34.1 yıl ve ortalama vücut kitle indeksi 43.6 kg / m2). Stapler hattından beş kaçak (% 2,6), yedi gastrointestinal kanama (% 3,7) vardı. Ölüm olmadı.
TARTIŞMA ve SONUÇ: Sleeve gastrektomi sırasında monopolar koter ile birlikte stapler hattı kanama kontrolü uygulanabilir, basit, ucuz ve nispeten güvenli bir yöntemdir. Monopolar koterin diğer stepler hattı kanama kontrol yöntemleriyle karşılaştırılması için randomize çalışmalar gereklidir.
